In my private practice, Dr. Sarah Jenkins, Ph.D., I am deeply committed to protecting the confidentiality of your Protected Health Information (PHI). I am required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to maintain the privacy of your PHI and to provide you with this Notice of my legal duties and privacy practices.

2. Uses and Disclosures of PHI

I will not disclose your clinical records to anyone without your explicit written authorization, except in the following situations required by law:

• Danger to Self or Others: If I believe you are an imminent danger to yourself or to a specific, identifiable other person.
• Abuse or Neglect: If I have reasonable suspicion of ongoing child abuse, elder abuse, or abuse of a dependent adult.
• Court Orders: If a judge issues a court order or subpoena requiring the release of your records.

3. Your Rights Regarding Your PHI

You have the right to request a copy of your clinical records, request amendments to your records if you believe they are inaccurate, and request an accounting of disclosures I have made of your PHI.

4. Website Data Collection

When you use my website, I do not collect any personal health information unless you voluntarily submit it through my secure intake form. The intake form uses AES-256 encryption. I do not use advertising trackers, marketing pixels, or third-party cookies that share your data with advertisers.
